No Period Causes and Treatment Options for Women in Mumbai

A missed period may occur because of pregnancy, but pregnancy is only one possible explanation. Menstrual cycles can also change because of stress, weight fluctuations, hormonal conditions, intense exercise, medication, chronic illness, breastfeeding, or the transition toward menopause.

The medical term for absent periods is amenorrhoea. Primary amenorrhoea describes periods that have not started by the expected age, while secondary amenorrhoea refers to periods stopping after they were previously present.

The First Step Is Usually a Pregnancy Test

For a woman who is sexually active and has missed a period, pregnancy should generally be checked first, even when contraception was used. Home urine tests can be useful, but testing too early may produce a negative result.

If the period remains absent, the test may need to be repeated or confirmed through medical evaluation. Pelvic pain, dizziness, shoulder pain, fainting, or bleeding with a positive pregnancy test requires urgent assessment because an ectopic pregnancy must be excluded.

Common Reasons Periods May Stop

Polycystic ovary syndrome

PCOS can cause irregular ovulation, infrequent periods, acne, unwanted hair growth, weight changes, or difficulty conceiving. Not every woman with PCOS has all these features.

Thyroid or prolactin disorders

Both low and high thyroid activity can affect menstrual cycles. Elevated prolactin may also suppress ovulation and can sometimes cause nipple discharge.

Stress and nutritional changes

Significant emotional stress, rapid weight loss, restrictive dieting, low body weight, or excessive exercise can disrupt hormonal signals between the brain and ovaries.

Perimenopause

Cycle changes become more common as women approach menopause. However, pregnancy remains possible until menopause is established, so new irregularity should not automatically be attributed to age.

Reduced ovarian function

Periods may stop when the ovaries are no longer releasing eggs regularly. When this happens before age 40, it requires evaluation for primary ovarian insufficiency.

Other possible causes include uterine scarring, chronic medical conditions, certain medicines, hormonal contraception, and breastfeeding.

What a Gynaecological Evaluation May Include

The doctor may ask about the last menstrual period, usual cycle length, sexual activity, contraception, pregnancy symptoms, medication, weight changes, exercise, stress, headaches, visual symptoms, and unwanted hair growth.

Depending on the history, evaluation may include:

  • Urine or blood pregnancy testing
  • Thyroid and prolactin tests
  • Reproductive hormone tests
  • Blood sugar assessment where indicated
  • Pelvic ultrasound
  • Additional tests based on examination findings

A single hormone result rarely provides the entire answer. Results need to be interpreted alongside symptoms and cycle history.

Treatment Should Address the Cause

No period causes and treatment options are closely connected. There is no universal tablet that is appropriate for every missed period.

PCOS management may involve lifestyle support, cycle regulation, protection of the uterine lining, or ovulation treatment when pregnancy is desired. Thyroid disorders require appropriate medical care. Nutritional or exercise-related amenorrhoea may improve when the underlying energy deficit is corrected. Women with reduced ovarian function may need counselling about bone health, hormonal support, and fertility.

Taking hormonal medicine without assessment may trigger bleeding without resolving the reason periods stopped. It can also delay the diagnosis of pregnancy or an underlying endocrine condition.

When to Arrange an Appointment

Medical advice is appropriate when:

  • Periods have stopped for three months without a known explanation
  • Cycles repeatedly occur more than 35–45 days apart
  • A teenager has not begun menstruating by the expected age
  • Missed periods occur with headaches or visual changes
  • There is pelvic pain or unusual bleeding
  • Pregnancy is desired but ovulation appears irregular
  • Menstrual changes occur with significant weight loss

Dr Bhavini Shah Balakrishnan provides menstrual evaluation for women in Mumbai, Maharashtra, using the patient’s history, examination, and selected investigations to identify the likely cause.

Periods are an important indicator of reproductive and general health. When they stop unexpectedly, identifying the underlying reason is more useful than simply trying to bring on temporary bleeding.
